BBL in Las Vegas: how fat grafting works and the risk facts

A Brazilian butt lift moves your own fat, taken by liposuction, to the buttocks. Some mommy makeover plans include it. It is also the cosmetic procedure surgical societies have issued the strongest safety warnings about, and anyone considering it in Las Vegas should understand why before choosing a surgeon. This guide sets out how it works, what the guidance says, and what to ask.

How a BBL works

Cleveland Clinic describes a BBL as fat removed by liposuction and then grafted into the buttocks. It is usually done under general anesthesia. The fat goes into the layer under the skin, called the subcutaneous layer, and not into the muscle. A BBL adds volume and shape. It does not lift sagging skin, which needs a different procedure.

Because the fat comes from liposuction, a BBL is in effect two procedures: everything that applies to liposuction applies here too, plus the risks of the grafting itself.

Why the safety guidance exists

The buttocks contain large blood vessels and nerves. Cleveland Clinic warns that if the cannula strikes them, it can lead to injury, a fat embolism or death. The task force below linked those deaths to fat placed in or under the muscle.

A task force of surgical societies, the American Society of Plastic Surgeons among them, estimated in January 2018 that roughly 1 in 3,000 gluteal fat grafting procedures ended in death, the highest rate it knew of for any aesthetic procedure. In every autopsy it reviewed, fat was found in or under the gluteal muscle. It reported that no autopsy had yet shown a death with fat only in the subcutaneous space.

What the 2018 task force advised

Graft fat only into superficial planes, with the subcutaneous space the safest; stage the procedure instead of going deep; inject only while the cannula is moving; and discuss the risk of death with every patient as part of consent.

In August 2022 ASPS, The Aesthetic Society and their foundations issued a joint statement. It supported the use of real-time ultrasound so the surgeon can see where the cannula is, and said BBLs in an office setting should be done only by surgeons with privileges for the procedure in a licensed surgery center or hospital. It also noted that Florida now requires the fat to be injected only above the gluteal fascia, limits surgeons to three BBLs a day and requires ultrasound. Those are Florida’s rules; this site has not found an equivalent Nevada law, so ask your Las Vegas surgeon which of these practices they follow.

Newer death-rate estimates exist, but this site could not verify them from the original source, so they are not quoted here. Ask your surgeon what current data they rely on.

What not to do: injections

The FDA warns against any dermal filler or liquid silicone injection to enlarge the buttocks. Injectable silicone is not approved for any aesthetic use. Treat any offer of buttock injections with that warning in mind.

Recovery, in general ranges

From Cleveland Clinic, for a BBL. Combined with a tummy tuck in a mommy makeover, recovery is longer and your surgeon sets your own plan.

  • No sitting or lying on the buttocks: at least two weeks.
  • Sitting only with cushion support: for about eight weeks.
  • Work: after about 10 to 14 days.
  • Heavy activity: not for about a month.
  • Full recovery: two to three months, when the surgeon usually judges the result.

Not sitting normally for weeks is hard with children: car journeys, meals and feeding all change. Plan how you will travel to follow-up visits, and who drives. Results can last several years or longer if your weight stays stable, Cleveland Clinic says.

What it costs

The American Society of Plastic Surgeons reports a 2024 national range of $7,000 to $11,500 for the surgeon’s fee alone for buttock augmentation with fat grafting. Anesthesia, the facility and other costs are extra. Las Vegas offices set their own prices; see the mommy makeover cost guide for how a combined quote is built.

ASPS counted 25,662 of these procedures in 2025, down 5% and, in its words, continuing a cooling trend.

How to choose a surgeon for a BBL

  1. Is your certification from the American Board of Plastic Surgery?
  2. Into which layer do you graft the fat, and how do you make sure it stays there?
  3. Do you use real-time ultrasound during grafting?
  4. Where is it done, and do you hold privileges for it at a licensed surgery center or hospital?
  5. Would you stage it rather than combine it with my other procedures?
  6. How many do you perform in a day?
  7. Will you go through the risk of fat embolism and death with me as part of consent?

Look up the Nevada license with the State Board of Medical Examiners, and check the ABPS certification search too. A surgeon who brushes off the safety questions is giving you an answer too. If your goal is mainly loose skin after weight loss, read the guide to body contouring.

Questions people ask

Is a BBL safe?

No surgery is free of risk, and this one carries serious risk. The 2018 task force of surgical societies estimated deaths at about 1 in 3,000 and advised grafting only under the skin, never into the muscle.

Where does the fat for a BBL come from?

From your own body, removed by liposuction from another area. Your surgeon explains which areas suit you.

Can a BBL be part of a mommy makeover?

ASPS lists buttock augmentation as one possible part. Ask whether your surgeon would combine it or stage it separately.

How long before I can sit normally?

Cleveland Clinic says no sitting on the buttocks for at least two weeks, and only cushion-supported sitting for about eight weeks.

Are buttock filler injections an alternative?

No. The FDA warns against filler or liquid silicone injections for buttock enlargement.

What does a BBL cost in Las Vegas?

Prices are set by each office. ASPS gives a 2024 national range of $7,000 to $11,500 for the surgeon’s fee alone, before anesthesia and the facility.
